e54c177118 Remove Analysis::into_engine.
This is a standard pattern:
```
MyAnalysis.into_engine(tcx, body).iterate_to_fixpoint()
```
`into_engine` and `iterate_to_fixpoint` are always called in pairs, but
sometimes with a builder-style `pass_name` call between them. But a
builder-style interface is overkill here. This has been bugging me a for
a while.

This commit:
- Merges `Engine::new` and `Engine::iterate_to_fixpoint`. This removes
  the need for `Engine` to have fields, leaving it as a trivial type
  that the next commit will remove.
- Renames `Analysis::into_engine` as `Analysis::iterate_to_fixpoint`,
  gives it an extra argument for the optional pass name, and makes it
  call `Engine::iterate_to_fixpoint` instead of `Engine::new`.

This turns the pattern from above into this:
```
MyAnalysis.iterate_to_fixpoint(tcx, body, None)
```
which is shorter at every call site, and there's less plumbing required
to support it.

6af470e360 Reduce visibilities, and add warn(unreachable_pub).
Lots of unnecessary `pub`s in this crate. Most are downgraded to
`pub(super)`, though some don't need any visibility.

84ac80f192 Reformat use declarations.
The previous commit updated `rustfmt.toml` appropriately. This commit is
the outcome of running `x fmt --all` with the new formatting options.

665821cb60 Add blank lines after module-level //! comments.
Most modules have such a blank line, but some don't. Inserting the blank
line makes it clearer that the `//!` comments are describing the entire
module, rather than the `use` declaration(s) that immediately follows.

df116ec246 Migrate memory overlap check from validator to lint
The check attempts to identify potential undefined behaviour, rather
than whether MIR is well-formed. It belongs in the lint not validator.

12b92c8a87 Visit only reachable blocks in MIR lint
No functional changes - all checks have been emitted conditionally on
block being rechable already.
